In this article the authors have compared the use of US guidance vs. conventional nerve stimulation for sciatic and femoral nerve blocks. I think one of the main advantages of using US guidance is the clear anatomical visualization of local anesthetic filling around the nerve with a potential for dose reductions. Further, more accurate determinations of adequacy of the blocks with potential motor block and sensory block determination may lead to an accurate utilization of this technique in routine practice. It will be important for us to continue to perform these studies, potentially in a multi-institutional format with many more patients to come up with accurate guidelines for pediatric usage. These will benefit the specialty of pediatric anesthesia greatly and should be pursued.
