We read with interest the case series by Gibbison et al about the i- gel and its ability to protect aspiration by identifying regurgitant material. We feel that this may be solely dependant on the supraglottic dead space created by the i-gel. Hence, despite the gastric channel, the identification of aspirate may be sooner if the supraglottic space is reduced. A design to have a closer fit of the i-gel may reduce this inherent problem.
