EditorThere is much to commend the anticipated guidance from the National Patient Safety Agency (NPSA) as outlined by Dr Cunliffe.1 Particularly to be welcomed is the advice regarding monitoring of serum electrolytes and restriction of the use of saline 0.18% with glucose. Where the NPSA has perhaps failed is with regard to what happens next. Its recommendation for maintenance (saline 0.9% with glucose 5%) has clearly been selected as
