Editor—We would like to report a case of a known but rare complication of metformin.

A 46-yr-old Caucasian female with schizophrenia was admitted from the mental health unit to the emergency department with a 2 day history of vomiting and abdominal pain. The nursing staff on the psychiatric ward reported she had not been eating or drinking properly for the past few days.
